Option 1: Anonymous Complaint Form
Use this form if you would like to raise a concern or complaint without providing your personal details. Your feedback will still be taken seriously, but we may not be able to contact you for follow-up.
Option 2: Participant/Client Complaint Form
For NDIS participants who wish to raise a complaint about the supports or services they receive. This includes where a family member or other advocate helps you to make a complaint.
Option 3: Complaint on Behalf of Someone
Use this form if you are submitting a complaint on behalf of an NDIS participant or another person receiving services. This includes parents, friends or family members, advocates or other service providers.
Option 4: Organisational Complaint Form
For non-participants, members of the public, other service organisations or professionals who wish to lodge a complaint regarding services, staff, or compliance issues.
