The purpose of this form is to allow individuals to raise concerns or complaints regarding the supports and services you have received through Flair For Care, under the NDIS framework. Submitting your complaint helps us to:
- Ensure safety and quality: We use complaints to review how services are delivered and to identify potential improvements. This aligns with the NDIS Practice Standards under Module 2: Provider Governance and Operational Management, which require providers to maintain a responsive and accessible complaints system.
- Resolve issues fairly and transparently: We aim to handle complaints effectively, respectfully, and promptly – following NDIS principles of procedural fairness and natural justice.
- Support continuous improvement: Your feedback is vital for improving our services and ensuring the best outcomes for NDIS participants. This supports the NDIS Commission’s expectations for provider complaint systems to be used for service improvement.
Who this form is for
This form is for anyone who wishes to raise a concern or complaint about the supports or services delivered by Flair For Care under the National Disability Insurance Scheme (NDIS). You may use this form if you are:
- An NDIS participant receiving supports or services from us.
- A family member, carer, or representative acting on behalf of a participant.
- A support worker, advocate, or community member who has witnessed an issue and wishes to raise it.
- Another service provider or organisation lodging a concern about our services or conduct.
